Driver License Required Documents

Federal and Country law requires proof of identity, Social Security number, and two (2) proofs of residential accost for a Florida driver license or ID bill of fare. Click here for a handy checklist of required documents.

ane. Identification (bring 1)

U.S. Citizens:

  • Original or certified U.Southward. birth certificate from a authorities agency

2. Proof of ALL Legal Name Changes (if whatsoever)

Ever changed your name?
If the name on your identification document doesn't match your current legal name, you will need to show proof of all legal name changes to show your current name.

  • U.Southward. passport in electric current proper name – OR-
  • Original or certified copies of ALL marriage certificates or court orders, such as divorce decrees, that show your proper name change(southward). Photocopies and non-certified copies are not valid.

3. Social Security number (bring 1)

U.S. citizens, immigrants, and not-immigrants required to have a social security number (SSN) must evidence acceptable proof of SSN and information technology must be verified with the Social Security Assistants (SSA) before a REAL ID driver license or ID card can be issued.

Notation: This must show your total Social Security number and total name.

  • Social Security carte du jour
  • W-2 course (manus-written not acceptable)
  • Paycheck stub showing applicant's name and total number (mitt-written non acceptable)
  • Internal Acquirement Service course 1099
  • U.S. Armed forces ID carte du jour showing Social Security number

Never been issued a Social Security number?
Individuals applying for an original, renewal, or replacement Existent ID driver license or ID card who have not shown proof of SSN or SSN ineligibility must show acceptable proof before ane can be issued – an affidavit from SSA volition non be accepted.

  • Non-immigrant/conflicting
    • Must show evidence of current non-work authorized not-immigrant status to be eligible
    • SSN is required for commercial commuter license (CDL) or commercial learner's permit (CLP)

4. Residential Address (bring two)

Notation: Each proof of address must show your consummate residential address. P.O. boxes are not accepted.

  • Human action, mortgage, monthly mortgage statement or payment booklet or rental agreement
  • Florida voter registration card
  • Florida vehicle registration or championship
  • Westward-2 form or 1099
  • Utility bill, claw up or work order
  • Automobile or homeowner's insurance policy or bill
  • Mail from fiscal institutions, including checking, savings, or investment account statements
  • Medical or health card with address
  • Alphabetic character from shelter or half-way house verifying applicant lives at this address
  • Educational transcripts

No proof of residential accost?

UPDATED AS OF JUNE 10, 2020:

Residing with a Relative
If you reside with a relative, you lot must provide two adequate proofs of residential address documents in the relative's proper name, document(s) showing the family relationship, along with a Certification of Address form to self-certify.

Acceptable family unit relationship documents may include, but are not limited to:

  • Union certificate
  • Birth document
  • Military machine dependent identification card
  • Adoption record

If you are unable to provide family relationship documents, a completed Certification of Address grade must be submitted.  The person you live with who is signing the form can choose to sign the document in our office at the fourth dimension of the transaction or sign the form in front of a notary.

Residing with a Non-Relative
If you lot reside with someone that is not a relative, y'all must provide two adequate proofs of residential address documents in the not-relative'southward name along with a Certification of Address form to self-certify.  The person you live with who is signing the course can choose to sign the document in our office at the time of the transaction or sign the form in forepart of a notary.
